Job summary

Evolve Therapy Services in Gretna, NE is seeking a compassionate Physical Therapy Assistant to support care in an in-house Skilled Nursing Facility on a PRN basis.

Under the supervision of a Physical Therapist, you will conduct screenings, deliver individual and group treatment, help develop treatment plans, and educate residents and families while ensuring CMS-compliant documentation.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluating under the direction of the Physical Therapist.
• Assist in developing target lists with short- and long-term goals and treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan, including strength, mobility, balance, gait, and ROM activities.
• Assist in selecting PT techniques/media and sequence of activities with supervision.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice

Ongoing continuing education and professional development
