Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house team on a PRN basis. You will administer prescribed PT treatments and help residents improve mobility and quality of life under PT supervision.

The role emphasizes documentation, teamwork, and effective communication with patients, families, and the interdisciplinary team. PRN scheduling allows flexible hours within a skilled nursing environment.

Qualifications

• Active Texas PTA license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System.
• Ability to work under Physical Therapist supervision.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients’ plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient responses and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Ensure documentation is thorough and timely per policies and regulations.
• Attend required meetings with the Director of Rehab.
