Job summary

Creative Solutions In HealthCare is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will provide person-centered care to help residents improve mobility and quality of life under supervision of a Physical Therapist.

Responsibilities include administering treatments, documenting progress, and collaborating with the interdisciplinary team. Texan PT license and SNF experience are preferred; new graduates welcome with a PRN schedule and

Qualifications

• Active and valid Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatments and documentation.
• Ability to collaborate with an interdisciplinary team.

Responsibilities

• Administer prescribed PTA treatments to improve mobility and function.
• Provide skilled PT services under PT supervision.
• Coordinate with PT to relay information and referrals.
• Implement and adjust care plans under PT supervision.
• Record motor function, strength, and progress accurately.
• Monitor patient response and adjust approaches with the team.
• Educate patients, families, and caregivers about care plans.
• Ensure timely documentation per policies and regulations.
• Attend meetings as designated by the Director of Rehab.
