Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate PT Assistant (PTA) to join the in‑house team on a PRN basis. You will support residents by delivering prescribed physical therapy to improve mobility, strength, and quality of life under PT supervision.

Responsibilities include administering treatments, recording progress, and collaborating with the interdisciplinary team. In‑house rehab and flexible schedules are offered, with SNF experience preferred and a valid Texas PTA

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• SNF or long‑term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain, improve or restore function.
• Provide skilled PT services under the supervision of a Physical Therapist.
• Relay information and referrals to the Physical Therapist on service requests.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust approaches with the team.
• Provide education to patients, families, and caregivers.
• Ensure documentation is thorough and timely per policy and regulations.
• Attend required meetings designated by the Director of Rehab.
