Job summary

Creative Solutions in Healthcare, Inc. seeks a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will provide person-centered care to improve mobility and quality of life for residents, under the supervision of a Physical Therapist.

The successful candidate will hold a Texas PTA license, have SNF or long-term care experience, be proficient with Net Health, and demonstrate strong communication and teamwork as part of an interdisciplinary team.

Qualifications

• Active Texas PTA license in good standing with licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health EHR or similar systems is a plus.
• Strong communication and teamwork skills are essential.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services under the supervision of a Physical Therapist.
• Relay information and referrals to the PT as needed.
• Implement and modify plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response to treatment and adjust approaches with the interdisciplinary team.
• Provide education to patients, family members, and caregivers.
• Ensure documentation is complete and compliant with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
