Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team.

The PTA will administer prescribed treatments to improve mobility, strength, and quality of life for residents under the supervision of a Physical Therapist. The role emphasizes person-centered care, collaboration with an interdisciplinary team, and accurate documentation to meet state and federal regulations.

Qualifications

• Active and valid Texas PTA license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health EDS preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments under PT supervision.
• Provide skilled PT services and interventions under physician orders.
• Record and relay information to the PT and interdisciplinary team.
• Implement and modify plans of care per PT supervision.
• Measure and document motor function, strength, and muscle performance.
• Ensure timely documentation per policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
