Job summary

Creative Solutions In HealthCare is seeking a compassionate PRN Physical Therapy Assistant (PTA) to join an in-house rehab team. You will assist residents in improving mobility and quality of life under PT supervision.

Responsibilities include administering prescribed treatments, documenting progress, and collaborating with the interdisciplinary team to adjust plans of care. Prior SNF experience is preferred, and Net Health documentation familiarity is a plus.

Qualifications

• Active and valid Texas PTA license.
• In good standing with regulatory agencies.
• SNF/long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health documentation.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled physical therapy services under supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients’ plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response to treatment interventions and modify approaches with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Ensure documentation is completed thoroughly and timely in compliance with company policies and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.
