Job summary

Creative Solutions in Healthcare, Inc. seeks a compassionate Physical Therapy Assistant (PTA) for a PRN role in our in-house rehab team.

You will administer PT treatments, assist under the supervision of a Physical Therapist, and help residents improve mobility and quality of life within a SNF setting. The ideal candidate holds an active Texas PTA license, maintains licensure, and collaborates closely with an interdisciplinary team.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under the supervision of a Physical Therapist.
• Relay information and referrals to the Physical Therapist on request.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength and muscle performance.
• Monitor patient response and adjust approaches with the team.
• Provide clinical support and education to patients, families and caregivers.
• Ensure documentation is thorough and timely per policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
