Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. This PRN position supports the Physical Therapist with resident care, screenings, and treatment planning.

Responsibilities include conducting group and individual therapies, coordinating schedules with the team, and ensuring compliant NetHealth documentation. Requires PTA licensure and ongoing continuing education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS/NetHealth documentation proficiency or willingness to learn.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluation planning and treatment plan development with the Physical Therapist.
• Carry out individual and group treatments focusing on strength, mobility, balance, gait, ROM, and modalities.
• Monitor resident responses and modify interventions as directed.
• Participate in scheduling with the Physical Therapist and care team.
• Document compliant and medically necessary notes in NetHealth EMR.
• Educate residents, families, and staff on therapy programs.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
