Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, Tennessee. This PRN position supports the Physical Therapist by delivering resident-centered therapy and participating in assessments and documentation.

Responsibilities include screenings, goal-oriented treatment planning, and collaboration with the care team. Licensure must be active in state(s) of practice and ongoing CEU requirements must be met.

Qualifications

• Must have an Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluations and in establishing treatment plans with goals.
• Deliver individual and group therapy per the treatment plan.
• Monitor resident progress and modify interventions as needed.
• Document CMS-compliant and medically necessary notes in NetHealth EMR.
