Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. This PRN position supports the therapy team under the supervision of a Physical Therapist to deliver high-quality resident care.

Responsibilities include screenings, treatment planning, and direct therapy, with emphasis on mobility, strength, balance, and safe transfers. Will participate in documentation and CMS-compliant records in NetHealth EMR.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and other team members.
• Document outputs and ensure CMS expectations are met in NetHealth EMR systems.
