Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. You will provide PT services under supervision, perform screenings, and support treatment plans to enhance resident mobility and function.

The role requires an active PTA license in the applicable state and a commitment to ongoing professional development. Full-time, part-time, or PRN options available with CMS-compliant documentation responsibilities.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation planning under the Physical Therapist's direction.
• Assist in developing short- and long-term goals and treatment plans.
• Conduct individual and group treatment focusing on strength, mobility, balance, gait, and ROM.
• Monitor resident response and modify interventions with PT consultation.
• Maintain resident schedules in coordination with the PT and care team.
• Document compliant and medically necessary CMS CMS expectations in NetHealth EMR.
