Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. This PRN position supports the resident-focused therapy program under the supervision of a Physical Therapist.

You will conduct screenings, assist in treatment planning, and help deliver individualized and group therapies to residents, while coordinating with the PT and care team. Ongoing licensure requirements and CMS documentation are part of the role.

Qualifications

• Associate's degree in physical therapy from an accredited program (required).
• Active Physical Therapy Assistant license in state(s) of practice (required).
• Ongoing continuing education to maintain licensure and certification (required).
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in identifying target goals and treatment plans.
• Conduct individual and group treatments as outlined in the plan (strength, mobility, gait, ROM, modalities).
• Assist in selecting PT techniques/media and sequence of activities with the PT guidance.
• Monitor residents' response and consult PT for modifications.
• Assist in scheduling with the PT and team members.
• Adhere to schedules and adjust as needed for treatment regimens.
• Train residents, family, and staff in equipment usage.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Report to work on time and optimize productivity during shifts.
• Participate in Quality Assurance as required.
• Complete compliant CMS-required documentation in NetHealth EMR.
