Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. The role supports resident care under a Physical Therapist, with opportunities for full-time, part-time, or PRN engagement.

The PTA will conduct screenings, assist in evaluations, and help implement treatment plans focusing on strength, mobility, balance, and gait. CMS documentation in NetHealth EMR is required.

Qualifications

• Requires an Associate's degree in physical therapy and an active PTA license.
• Experience working in a skilled nursing facility is beneficial.
• Ability to work under supervision and follow a physical therapist's plan.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in evaluations and in identifying goals and treatment plans.
• Deliver individual and group therapy focusing on strength, mobility, balance, and ROM.
• Assist in selecting techniques and adapting treatment based on patient response.
• Document compliant and medically necessary CMS documentation in NetHealth EMR.
• Coordinate resident schedules with the Physical Therapist and team.
• Support resident and family education and staff training.
