Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. The role operates PRN and works under a Physical Therapist to deliver resident-focused therapy services on-site.

You will conduct screenings, assist with evaluations, and implement treatment plans, while ensuring accurate CMS-compliant documentation in NetHealth EMR and coordinating with the care team for optimal resident outcomes.

Qualifications

• Must have an Associate's degree in physical therapy and a valid PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluating residents under the Physical Therapist's direction.
• Develop target goals and treatment plans with the Physical Therapist.
• Carry out individual and group treatments as outlined in the plan.
• Support in selecting appropriate PT techniques/media and sequence of activities.
• Monitor resident responses and adjust as advised by the PT.
• Assist in scheduling resident therapy sessions with the PT and team.
• Ensure adherence to schedules and modify as needed for treatment.
• Educate residents, families, and staff on therapy use and safety.
• Maintain thorough documentation per CMS expectations in NetHealth EMR.
