Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. This PRN position supports the therapy team under the supervision of a Physical Therapist.

Responsibilities include resident screenings, treatment planning support, and delivering both individual and group therapy focusing on mobility, strength, gait, and functional activities. Excellent CMS documentation is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in developing target goals and treatment plans with the PT.
• Deliver individual and group treatment focusing on strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adapt as needed.
• Monitor response to intervention and consult PT for modifications.
• Maintain resident schedules and ensure timely treatment.
• Train residents, families, and staff in use of equipment.
• Document compliant and medically necessary CMS documentation in NetHealth EMR.
