Job summary

Evolve Therapy Services, LLC in Le Center, MN is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy and help residents achieve mobility and independence.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in evaluation and goal setting with the Physical Therapist.
• Develop and modify treatment plans with targets for short- and long-term goals.
• Perform individual and group treatment focusing on mobility, strength, balance, and gait.
• Document interventions in NetHealth EMR in accordance with CMS expectations.
• Coordinate schedules and participate in quality assurance activities.
