Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility located in Le Center, MN. The ideal candidate will assess residents, develop treatment plans, and work with an interdisciplinary team to promote mobility and quality of life.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents per standards and facility procedures.
• Maintain accurate documentation and billing per CMS expectations in NetHealth EMR.
• Conduct screenings and evaluations to guide treatment plans.
• Develop and implement short- and long-term goals for residents.
• Interpret evaluation findings to residents, families, and the care team.
• Collaborate with interdisciplinary team to optimize resident care.
