Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. This PRN position supports residents under the supervision of a Physical Therapist and requires a Minnesota PTA license and an accredited degree.

Responsibilities include screenings, goal setting, individual and group treatment, and documentation in NetHealth EMR. The role emphasizes ongoing education, adherence to CMS standards, and collaboration with the care team.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan that includes areas of muscle strength, coordination, mobility, balance, gait, ROM, and modalities.
• Assist in selecting PT techniques/media and sequence of activities, adapting as indicated by resident performance in consultation with the Physical Therapist.
• Monitor residents’ response to intervention and consult with the Physical Therapist if modification is indicated.
• Establish and modify individual resident schedules in conjunction with the Physical Therapist and other team members.
• Adhere to department and resident schedule and modify as appropriate for treatment regimen.
• Train resident, family and staff in use of equipment.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Report to work on time and coordinate schedule for maximum productivity during assigned shift.
• Assist with Quality Assurance Program as required.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR.
