Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for our partner in Le Center, MN, to support in-house skilled nursing facility services. The role involves supervising under a Physical Therapist, conducting screenings, and assisting in evaluations to create effective treatment plans.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and development of targeted goals and treatment plans.
• Deliver individual and group treatments addressing strength, mobility, gait, balance, and ROM.
• Document CMS-compliant, medically necessary information in NetHealth EMR.
• Educate residents, families, and staff on treatment plans and equipment use.
