Qualifications

• Bachelor’s degree in Physical Therapy required; Masters or Doctorate preferred.
• Licensed to practice Physical Therapy in the state; continuing education requirements must be met.
• Proficient computer skills for data entry in Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and staff in resident care activities.
• Communicate progress and discharge plans with physicians, families, and nursing staff.
• Maintain accurate documentation in medical records per facility and payer requirements.
