Job summary

Qualifications

• Bachelor's degree in physical therapy required.
• Less than 1 year of relevant experience required.
• Licensed Physical Therapist upon hire; CPR/BLS required.

Responsibilities

• Evaluates patient using PT assessment tools.
• Plans individualized physical therapy program including exercises and modalities.
• Directs and supervises patient activities and ensures safe handling.
• Coordinates with the interdisciplinary team to meet goals.
• Provides patient education for ongoing care.
• Maintains accurate medical documentation.
