Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis in Texas. You will evaluate residents, plan ADL/IADL–focused therapy, and adjust care to improve safety and independence.

You will collaborate with an interdisciplinary team, document meticulously, and provide family education while ensuring compliance with state and federal regulations. New grads welcome to apply.

Qualifications

• Active Texas OT license required.
• SNF or long-term care experience preferred.
• Strong communication and collaboration skills.

Responsabilités

• Evaluate patients and assess ADLs/IADLs.
• Develop individualized treatment plans and monitor progress.
• Document thoroughly and timely per policy.
• Collaborate with interdisciplinary rehab team.
