Job summary

Creative Solutions In HealthCare is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis in Texas. The role focuses on improving residents’ independence and safety through meaningful daily activities and functional outcomes.

You will evaluate patients, develop and adjust treatment plans, coordinate with an interdisciplinary team, document thoroughly, and support families and caregivers.

Qualifications

• Must hold an active Texas OT license.
• SNF or long-term care experience preferred.
• Knowledge of current occupational therapy treatment practices.
• Experience with Net Health EHR systems preferred.

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications as needed.
• Deliver treatments per the plan of care and monitor responses.
• Document thoroughly and timely per policies and regulations.
• Collaborate with the rehab team to ensure high-quality care.
