Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will support residents' independence and safety through meaningful activities and ADLs/IADLs.

Active Texas OT license required; SNF or long-term care experience preferred. Flexible schedules and in-house rehab model available. New graduates welcome to apply.

Qualifications

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily activities
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans to maximize performance in ADLs and IADLs
• Assess home environments and recommend modifications
• Recommend adaptive equipment and train patients for independence and safety
• Ensure treatments follow the plan of care
• Monitor patient response and modify interventions with the care team
• Provide clinical education and support to patients, families, and caregivers
• Collaborate with interdisciplinary team to ensure high-quality care
• Complete documentation in compliance with policies and regulations
• Comply with all applicable rules and procedures
• Attend required meetings designated by the Director of Rehab
