Job summary

Creative Solutions in Healthcare - East seeks a compassionate Occupational Therapy Assistant (OTA) to join our in-house rehab team. This full-time or PRN position provides person-centered care to help residents improve independence and quality of life.

You will plan and administer OT treatments under the supervision of an Occupational Therapist, document thoroughly, and collaborate with families and caregivers to support positive outcomes. Texas licensure required; SNF experience preferred.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• COTA credential required.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Plan and administer occupational therapy treatments to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist.
• Respond to requests for service by relaying information and referrals to the Occupational Therapist.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response to treatment interventions and report changes as appropriate.
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.
