Job summary
Creative Solutions in Healthcare, Inc. is seeking a compassionate Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will provide person-centered care to residents, helping them regain independence and improve quality of life under supervision.

Qualifications
- Must hold an active Texas OTA license and COTA credential.
- SNF/long-term care experience preferred.
- Proficient in OT treatment practices and state regulations.
- Experience with Net Health Electronic Documentation System preferred.
Responsibilities
- Plan and administer occupational therapy treatments to residents and patients to relieve pain, restore or improve function, and promote healing.
- Provide skilled occupational therapy services under the supervision of an OT.
- Respond to requests for service by relaying information to the OT.
- Ensure all treatments align with the established plan of care.
- Provide clinical support to patients, families, and caregivers.
