Job summary

Qualifications

• Active Texas OT license.
• SNF/long-term care experience preferred.

Responsibilities

• Assess home environments and recommend modifications.
• Provide training on adaptive equipment for independence and safety.
• Document thoroughly per plan of care and regulations.
• Collaborate with interdisciplinary team on patient care.
• Monitor progress and adjust interventions as needed.
• Educate patients, families, and caregivers.
• Attend required Rehab Department meetings.
