Job summary

Creative Solutions in Healthcare, Inc. in Midland, TX is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis.

You will focus on improving residents' independence, safety, and quality of life through meaningful ADLs and IADLs, aligned with an established plan of care. The ideal candidate holds Texas OT license, SNF or long-term care experience, and strong communication skills to collaborate with the interdisciplinary team.

Qualifications

• Active Texas Occupational Therapy license in good standing.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily activities.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Document thoroughly and timely according to plan of care and policies.
• Collaborate with interdisciplinary team and participate in meetings.
