Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis.

You will enhance residents’ independence and safety through meaningful daily activities and functional outcomes, working with an interdisciplinary team in a SNF setting. The ideal candidate holds a valid Texas OT license, has experience in long-term care, and is skilled in ADLs and IADLs.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with all regulatory agencies and licensing boards.
• Skilled Nursing Facility (SNF) or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home and living environments; recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments in accordance with the established plan of care.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide education and support to patients, families, and caregivers.
• Collaborate with team to ensure high-quality patient care.
• Complete documentation thoroughly and timely per policies and regulations.
• Attend required meetings designated by the Director of Rehab.
