Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You’ll evaluate residents, develop individualized ADL/IADL treatment plans, and collaborate with the interdisciplinary team to maximize independence and safety.

Texas license required with SNF or long-term care experience preferred; familiarity with Net Health documentation is a plus. Flexible schedules and patient-centered care are hallmarks of this role.

Qualifications

• Active and valid Texas OT license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications.
• Provide training on adaptive equipment.
• Deliver treatments per care plan and ensure compliance.
• Monitor response to therapy and adjust interventions with the team.
• Complete documentation thoroughly and timely per policies.
