Job summary

St Giles Nursing and Rehabilitation Center seeks a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, develop personalized treatment plans, and collaborate with an interdisciplinary team to improve ADLs and IADLs in a SNF setting.

You will work with patients to adapt environments and equipment, document findings, and educate families while maintaining compliance with regulations.

Qualifications

• Active and valid Texas Occupational Therapy license
• SNF or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations
• Develop individualized treatment plans to maximize ADLs and IADLs
• Assess home environments and recommend modifications
• Provide training on adaptive equipment to support independence and safety
• Ensure treatments align with the plan of care and state/federal regulations
• Monitor patient response and adjust interventions in collaboration with the team
• Provide education and support to patients, families, and caregivers
• Document thoroughly and timely per company policies
