Job summary

Creative Solutions in Healthcare - West is seeking an experienced Occupational Therapist (OT) to join our in-house team on a PRN basis. You will focus on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

You will collaborate with an interdisciplinary team, evaluate patients, and develop individualized treatment plans for ADLs and IADLs, with flexible scheduling and strong emphasis on patient-centered care.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend safety modifications.
• Recommend adaptive equipment and provide training.
• Deliver treatments per the plan of care and monitor response.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete documentation thoroughly and timely per policies and regulations.
