Innovate Rehab and Wellness is seeking a licensed Occupational Therapist for a Skilled Nursing Facility on a PRN, as-needed basis. You will evaluate patients’ functional abilities, develop individualized treatment plans, and guide recovery to restore independence and quality of life.

You’ll collaborate with physical therapy, speech-language pathology, physicians and the full care team, document progress, and stay current with OT best practices.

Qualifications

• Master's or Doctorate in OT from an accredited program.
• CPR certification preferred.
• Experience as an OT is advantageous but new grads welcome.

Responsabilités

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.
