Job summary

Responsibilities

• Conducts assessments and reassessments of the patient’s level of function.
• Plans, implements, and supervises an individualized therapeutic program to restore function and independence.
• Coordinates with Rehab Director to integrate the therapeutic program into the plan of care.
• Instructs patients and families in therapeutic activities for improving function and independence in ADLs.
• Documents patient progress and changes in condition or treatment and communicates findings.
