Responsibilities

• Conduct thorough assessments with personalized, comprehensive care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) as needed.
• Coordinate schedules with patients and plans of care.
• Monitor and report changes in patient symptoms promptly.
• Administer medications and treatments as prescribed by physician.
• Communicate with Case Managers and physicians to ensure unified care.
• Maintain accurate patient documentation in the EMR.
• Provide emotional support to patients and families.
