Qualifications

A valid and active RN license in the state of practice.
Willingness to travel, bringing care directly to patients' homes.
Graduation from an accredited school of nursing.
Current BLS certification.
Ability to pass a state & federal background check.
Malpractice insurance is required.
Strong problem-solving and empathetic approach.

Responsibilities

Conduct thorough assessments to provide personalized, comprehensive care.
Educate patients and families on health maintenance and disease prevention.
Assist with ADLs with dignity and respect.
Coordinate care with patients’ plans, hours, and preferences.
Monitor changes in patient symptoms and report promptly.
