Job summary

Saint Agnes Home Health and Hospice is seeking a Hospice Registered Nurse (RN) for part-time work in California. The role involves assessments, care planning, and coordination with an interdisciplinary team to deliver compassionate, home-based care that aligns with regulatory standards.

Qualifications include a CA RN license and at least 1 year of nursing experience, with home health experience preferred. Travel and a valid driver's license are required.

As a Hospice Registered Nurse (RN), you will apply critical thinking and clinical expertise to perform comprehensive assessments, develop personalized care plans, and deliver high-quality, compassionate care. Your goal is to maximize the comfort, dignity, and well-being of patients and their families in alignment with the interdisciplinary plan of care and all regulatory standards. In the role of Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

What You’ll Do

What You Bring

• Current RN license (BSN preferred)
• Minimum 1 year of nursing experience (required in California)
• Home healthcare experience preferred
• Valid driver’s license and reliable transportation

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
• Occasionally bend, stoop, twist, and assist with patient positioning (10%)
