Job summary

Saint Agnes Home Health and Hospice, part of Trinity Health At Home, seeks a Hospice Registered Nurse (RN) to perform comprehensive assessments, develop personalized care plans, and deliver compassionate, patient-centered hospice care at home in California.

The Hospice Admission RN will initiate the hospice plan of care, ensure timely communication, and coordinate with the interdisciplinary team to maximize comfort, dignity, and well-being for patients and their families.

Qualifications

• Current RN license (BSN preferred).
• Minimum 1 year nursing experience (California requirement).
• Home healthcare experience preferred.
• Valid driver’s license and reliable transportation.
• Strong computer skills and commitment to mission-driven care.

Responsibilities

• Provide skilled nursing care per physician orders and policies.
• Perform comprehensive assessments and develop individualized care plans.
• Collaborate with physicians, therapists, social workers, and aides.
• Supervise and support LPNs and Home Health Aides.
• Ensure regulatory compliance and contribute to quality improvement.
• Promote patient satisfaction and positive health outcomes.

What You’ll Do

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
