Provide skilled nursing care in accordance with physician orders and organizational policies
Perform comprehensive assessments and develop individualized care plans
Collaborate with physicians, therapists, social workers, and home health aides
Supervise and support LPNs and Home Health Aides
Ensure compliance with regulatory standards and contribute to quality improvement initiatives
Promote patient satisfaction and positive health outcomes
Current RN license (BSN preferred)
Minimum 1 year of nursing experience (required in California)
Home healthcare experience preferred
Valid driver’s license and reliable transportation
Strong computer skills and a commitment to ethical, mission‑driven care
