Job summary

Saint Agnes Home Health and Hospice in California is seeking a Hospice Registered Nurse (RN) to perform comprehensive assessments, develop care plans, and deliver compassionate care at home. You will coordinate with an interdisciplinary team and initiate the hospice plan of care in line with regulatory standards.

Minimum 1 year of nursing experience (California required) with a valid driver's license. BSN preferred.

Qualifications

• Active RN license in California (BSN preferred).
• Minimum 1 year of nursing experience (California required).
• Home healthcare experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care per physician orders and policies.
• Perform comprehensive assessments and create individualized care plans.
• Collaborate with physicians, therapists, social workers and aides.
• Supervise and support LPNs and Home Health Aides.
• Ensure regulatory compliance and contribute to quality initiatives.
• Promote patient satisfaction and positive health outcomes.

As a Hospice Registered Nurse (RN), you will apply critical thinking and clinical expertise to perform comprehensive assessments, develop personalized care plans, and deliver high-quality, compassionate care. In the role of Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

What You Bring

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
