Saint Agnes Home Health and Hospice in California is seeking a Primary Care Nurse / Registered Nurse to cover the Fresno area. This role delivers compassionate, at-home care with a focus on quality outcomes and patient satisfaction.

Qualifications

Responsibilities

• Provide skilled nursing care in accordance with physician orders and organizational policies.
• Perform comprehensive assessments and develop individualized care plans.
• Collaborate with physicians, therapists, social workers, and home health aides.

• Ensure compliance with regulatory standards and contribute to quality improvement initiatives.
