Job summary

Saint Agnes Home Health and Hospice in California is seeking a part-time Hospice Registered Nurse (RN) to apply clinical expertise in home health care, develop personalized care plans, and deliver compassionate care in alignment with interdisciplinary plans of care.

The role emphasizes initiating hospice plans of care, clear communication, and adherence to regulatory standards. Requires an active California RN license, 1+ year nursing experience, and willingness to travel for home visits.

Qualifications

• Current RN license; BSN preferred.
• Minimum 1 year of nursing experience (required in California).
• Home healthcare experience preferred.

Responsibilities

• Provide skilled nursing care per physician orders and policies.
• Perform comprehensive assessments and develop individualized care plans.
• Collaborate with physicians, therapists, social workers, and home health aides.
• Supervise and support LPNs and Home Health Aides.
• Ensure regulatory compliance and contribute to quality improvement.

What You’ll Do

• Provide skilled nursing care in accordance with physician orders and organizational policies

• Perform comprehensive assessments and develop individualized care plans

• Collaborate with physicians, therapists, social workers, and home health aides

• Supervise and support LPNs and Home Health Aides

• Ensure compliance with regulatory standards and contribute to quality improvement initiatives

• Promote patient satisfaction and positive health outcomes

What You Bring

• Current RN license (BSN preferred)

• Minimum 1 year of nursing experience (required in California)

• Valid driver’s license and reliable transportation

• Strong computer skills and a commitment to ethical, mission‑driven care

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
