LiveWell Partners’ Hope Home Care Network is seeking a licensed Physical Therapist to evaluate patients in home care settings and develop/oversee care plans. The PT delivers therapy services and supervises assistants within a collaborative interdisciplinary team.

Company: Hope Home Care - a LiveWell Partners Network Agency
Pay per visit, $64 - $180 per visit (amount dependent upon visit type), $384 - $600 per day, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

The Physical Therapist (“PT”) is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.
