LiveWell Partners is seeking a Home Health Physical Therapist to evaluate patients at home, formulate treatment plans, and oversee PT assistants within an interdisciplinary care team.
This PRN role requires at least 1 year of PT experience, an active license, CPR certification, and a valid driver’s license; a minimum of 6 visits per week with flexible scheduling.
Pay is per visit ($74–$180) with mileage reimbursement; payroll biweekly and opportunities for professional growth.
Pay per visit, $74 - $180 per visit, Visit rates dependent upon visit type, Mileage reimbursement at $0.55 per mile, Payroll processed biweekly (26 pay periods).
PRN (as needed), Minimum availability of 6 visits per week, Flexible schedule.
The Physical Therapist ("PT") is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.
