You will work with the care team to create and adjust treatment plans and document all care comprehensively.

Qualifications

• Active state PT license is required.
• CPR certification is required.
• Minimum 1 year PT experience; preference for 2+ years in acute or post-acute settings.
• Valid driver's license is required.

Responsibilities

• Determine PT treatment plans in collaboration with the care team and physicians.
• Provide and document all PT care and supervise aides as needed.
• Administer PT treatments as ordered by a physician.
• Evaluate progress and adjust plans as necessary.
• Complete discharge planning with care team and physicians for continued care post-discharge.
• Participate in weekly case conferences and communicate effectively across disciplines.

PRN Home Health Physical Therapist (PT)

Company: Hope Home Care - a LiveWell Partners network agency

Compensation: Pay per visit, $64 - $180 per visit (amount dependent upon visit type), $384 - $600 per day, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Shift Type: PRN (as needed), Minimum availability for 6 visits per week, Flexibility, In the field/On the road.

As a PRN Home Health Physical Therapist (PT), you will play a crucial role in evaluating and treating patients in their homes, delivering top-quartile clinical outcomes and exceptional patient experiences. Responsibilities:

• Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.

• As a Home Health Physical Therapist (PT) provide and complete all proper documentation.

• Administer physical therapy treatments as ordered by a physician, directing and overseeing treatment given by physical therapy assistants.

• Evaluate the effects of physical therapy treatments by observing, noting, and evaluating patients’ progress, and recommending adjustments and modifications.

• Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge.

• Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.

Qualifications:

• Minimum of 1 year of physical therapy experience, with a preference for 2+ years in an acute or post-acute care setting.

• Active state licensure as a Physical Therapist (PT) is required.

• Proof of current CPR certification.

• Valid driver's license.
