Job summary

LiveWell Partners seeks a Physical Therapist to evaluate patients in the home care setting, develop treatment plans, and supervise assistants. You will collaborate with the care team, physicians, and other stakeholders to ensure optimal patient outcomes.

The role is full-time and field-based, with 8 AM–4 PM hours, travel to patient homes, and a competitive benefits package including premium coverage, 401(k), and paid holidays.

Qualifications

• Minimum of 2 years of physical therapy experience preferred (2+ years in acute or post-acute care).
• Active state licensure in physical therapy required.
• Proof of current CPR certification and valid driver’s license.
• Proof of current auto insurance coverage.

Responsibilities

• Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
• Provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician. Direct and oversee treatment given by physical therapy assistants.
• Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients’ progress, and recommending adjustments and modifications.
• Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and/or providing equipment; recommending outpatient follow-up programs).
• Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.

Compensation:

Guaranteed base Salary, Per point incentive pay in addition to the base salary, Per point value between $68 and $75, Payroll processed bi-weekly (26 pay periods), Mileage reimbursed at $0.67/mile (in accordance with IRS guidelines of reimbursement).

Shift Type:

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

The Physical Therapist (“PT”) is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.
