Job summary

Hope Home Care - a LiveWell Partners network agency is seeking a skilled Home Health Physical Therapist (PT) to join our field team. As a PRN provider, you will evaluate and treat patients in their homes, code documentation, and collaborate with the care team to deliver high-quality outcomes.

You will work on a flexible schedule with a minimum of six visits per week, driving between patient homes while adhering to clinical guidelines and documentation standards.

Qualifications

• Minimum of 1 year of physical therapy experience, preference for 2+ years in acute or post-acute care.
• Active state licensure as a Physical Therapist (PT) is required.
• Proof of current CPR certification.
• Valid driver's license.

Responsibilities

• Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
• As a Home Health Physical Therapist (PT) provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician, directing and overseeing treatment given by physical therapy assistants.
• Evaluate the effects of physical therapy treatments by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.
• Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge.
• Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.

Company: Hope Home Care - a LiveWell Partners network agency

Compensation: Pay per visit, $64 - $180 per visit (amount dependent upon visit type), $384 - $600 per day, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Shift Type: PRN (as needed), Minimum availability for 6 visits per week, Flexibility, In the field/On the road.
