Job summary

Corewell Health is seeking a PRN/Casual Social Worker in Taylor, MI to support discharge planning and care coordination at Taylor Hospital. You will assess psychosocial needs, coordinate with community resources, and document plans in the EMR for a smooth transition of care.

The role collaborates with care teams and state and federal regulations are observed. Requires a Master’s in Social Work and Michigan licensure upon hire; prior experience in crisis intervention and bereavement support is

Qualifications

Required Master’s Degree in Social Work.
Experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

Conduct psychosocial assessments with patient and/or family to identify needs and plan post-discharge care.
Coordinate with alternate sites of care and community agencies to address patient needs.
Document assessment and plan in EMR reflecting current condition and transition of care needs.
Provide consultation and resources to healthcare team.
Maintain knowledge of community resources.
Participate in department/regional/state stakeholder committees as assigned.
Stay current on federal/state regulations pertaining to the role.
