Corewell HealthCare Management – Taylor Hospital seeks a licensed MSW to support patients and families with complex psychosocial needs, focusing on discharge planning and care transitions.

You will conduct psychosocial assessments, coordinate with alternate care sites and community resources, provide counseling, and help navigate federal/state regulations. This is a part-time, day-shift role with variability, embedded in an interdisciplinary healthcare team.

Qualifications

• Master's degree in Social Work (MSW) required.
• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MSW licensure in Michigan upon hire required.

Responsibilities

• Conduct psychosocial assessments with patients and families to identify needs and plan discharge.
• Coordinate with alternate sites of care and community agencies to address patient needs.
• Document assessment and plan in the electronic medical record reflecting current condition and transition needs.
