Qualifications

Responsibilities

• Collaborate with an interdisciplinary team to assist patients and families with psychosocial needs.
• Perform psychosocial assessments and develop post-discharge care plans.
• Coordinate with alternate care sites and community resources.
• Document assessments and plans accurately in the EMR.
• Provide consultation and resources to the healthcare team.
• Maintain current knowledge of community resources and regulations.
• Participate in department, regional, or statewide committees as assigned.
